Scientists reviewed the application of chimeric antigen receptor (CAR) T cell therapy against senescent cells in a paper published in Research doi.orgResearchdoi.orgOpen source ↗. The work examines the transition of cancer immunotherapy concepts to aging-associated pathologies. The authors analyzed preclinical data on specific cell-surface targets, genetic engineering strategies, and physiological outcomes across animal models of fibrosis and organ dysfunction.

Senescent cells cease division while secreting pro-inflammatory and profibrotic factors known as the senescence-associated secretory phenotype (SASP). First-generation small-molecule senolytics face limitations such as transient efficacy, poor tissue distribution, and modest target specificity. By contrast, engineered CAR T cells provide antigen-directed cytotoxic activity and in vivo expansion capabilities.

Preclinical target validation

Preclinical studies evaluated multiple cell-surface antigens, including uPAR, NKG2D ligands, FAP, PDGFR, and CCR2. In liver fibrosis models, single-cell analysis showed over 60% co-expression of uPAR with p16. A single dose of uPAR-targeting CAR T cells reduced senescent cells by 87% and liver collagen deposition by 52% in mice, while also improving metabolic markers and exercise tolerance. For NKG2D, ligands such as MICA showed approximately twofold higher expression on senescent cells, and NKG2D-directed CAR T cells selectively cleared damaged cells induced by various stressors. Additionally, CAR T constructs targeting FAP and PDGFRβ reduced collagen volume and improved cardiac ejection fraction and renal performance in animal models.

Engineering logic circuits and safety controls

Because senescent cells play active physiological roles in wound healing and tissue repair, indiscriminate elimination presents risks of off-target toxicity and cytokine release syndrome. To restrict targeting to pathological cells, investigators propose multi-antigen synNotch AND logic gates along with switchable suicide systems, including iCasp9 and HSVTK cassettes doi.orgResearchdoi.orgOpen source ↗. Manufacturing adaptations for cells derived from aged donors include selecting naive CD45RA+CCR7+CD62L+ phenotypes, employing IL-15 and IL-7 cytokine supplementation, and applying TERT mRNA electroporation, which extended telomeres by approximately 0.9 kb across 43 population doublings. The authors emphasize that all evaluated anti-senescence CAR T therapies remain strictly at the preclinical stage.
