Researchers from Soochow University, Zhejiang University, and Nanyang Technological University have developed a pathology-adapted peptide-oligonucleotide nanohybrid (PON) designed to counteract glucocorticoid-induced bone loss (GIBL) [1]. Glucocorticoid exposure often triggers systematic osteopenia and osteonecrosis of the femoral head, presenting a clinical challenge due to the simultaneous deterioration of bone architecture and local microvasculature [1]. As reported in Nature Communications, the targeted modular delivery platform was engineered to deliver microRNA-26a-5p directly into injured osseous environments [1].

## Targeted Nanohybrid Architecture

The PON design incorporates several functional features, including molecular self-assembly, affinity for bone tissue, enzyme-triggered responsiveness, cell-penetrating capabilities, and endosomal-lysosomal escape mechanisms [1]. Together, these characteristics overcome traditional systemic transport barriers associated with oligonucleotide therapeutics, shielding the cargo and improving localization within glucocorticoid-damaged microenvironments [1]. Systemic distribution of native oligonucleotides typically suffers from rapid degradation and low target-tissue bioaccumulation, issues addressed by this integrated delivery system [1].

## Molecular Pathways and In Vivo Efficacy

Mechanistically, the targeted delivery of miR-26a-5p suppressed glycogen synthase kinase-3 beta (GSK-3β) expression and promoted the stabilization of hypoxia-inducible factor 1-alpha (HIF-1α) [1]. This dual effect sustained osteogenesis-angiogenesis coupling while reducing cellular apoptosis under glucocorticoid exposure [1]. In rat disease models, systemic administration of the nanohybrids protected against initial skeletal and vascular degradation and limited pathological progression in established conditions [1].
