A pooled analysis of individual patient-level data from 506 adults across three randomized trials showed that ultrasound renal denervation significantly lowered blood pressure compared to a sham procedure [1]. The findings, published in JAMA Cardiology by researchers led by Columbia University, combined data from the RADIANCE-HTN SOLO, RADIANCE-HTN TRIO, and RADIANCE II trials evaluating the ReCor Medical Paradise System.

The study enrolled 506 participants with a mean age of 54.1 years, including 354 males, who presented with uncontrolled hypertension ranging from mild-to-moderate without medications to treatment-resistant forms on a standardized triple-drug therapy. After a 1-month medication stabilization phase, baseline daytime ambulatory systolic blood pressure was 150.3 mm Hg in the ultrasound cohort and 150.8 mm Hg in the sham group. Patients and treating physicians remained blinded to group assignments throughout the protocol.

## Blood pressure reduction

At the 2-month primary endpoint, daytime ambulatory systolic blood pressure dropped by 8.5 mm Hg to 141.8 mm Hg in the ultrasound renal denervation arm, compared with a 2.9 mm Hg decrease to 147.9 mm Hg in the sham arm [1]. This resulted in an adjusted between-group mean difference of -5.9 mm Hg favoring the intervention. Similar reductions were documented across other tracking methods: office systolic blood pressure fell by 10.4 mm Hg versus 3.4 mm Hg (mean difference -6.4 mm Hg), and home systolic blood pressure decreased by 8.4 mm Hg versus 1.4 mm Hg (mean difference -6.8 mm Hg).

## Safety and response predictors

The ultrasound catheter targets renal nerves along the renal arteries to decrease neurohormonal signaling that regulates vessel tension and fluid balance. Higher baseline blood pressure, elevated baseline heart rate, and orthostatic hypertension were identified by researchers as independent predictors of a larger blood pressure reduction following the intervention. Across the monitored cohort, no differences in early safety endpoints were observed between the denervation and sham groups [1].
